Why Sinus and Nasal Problems are So Prevalent in Islamabad & Rawalpindi
Living in the Islamabad and Rawalpindi twin cities comes with significant environmental challenges. With extreme seasonal pollen bursts (especially Paper Mulberry in spring), high particulate air pollution, dust, and dry winter weather, thousands of residents suffer from chronic nasal blockage, sinus headaches, and persistent post-nasal drip.
While occasional allergic flare-ups respond well to saline rinses and oral antihistamines, a large percentage of individuals experience chronic daily symptoms: difficulty breathing through one or both nostrils, heavy facial pressure behind the eyes, snoring, reduced sense of smell, and unrestful sleep.
When symptoms persist for longer than 12 weeks despite multiple courses of antibiotics and nasal steroid sprays, the problem is no longer just "allergies." It is an anatomical blockage. Prof. Dr. Maqbool Ahmed Baloch explains how modern minimally invasive endoscopic procedures provide a permanent cure.
The Two Culprits: Deviated Septum (DNS) & Sinus Ostium Blockage
1. Deviated Nasal Septum (DNS) & Inferior Turbinate Hypertrophy
The nasal septum is the cartilage and bone wall that divides your left and right nasal cavities. In over 70% of people, the septum is off-center or bent (deviated). When severe, DNS severely restricts airflow through one side of the nose.
To compensate, the swollen fleshy structures inside the opposite nostril (the inferior turbinates) swell continuously—a condition known as compensatory hypertrophy. The result is chronic two-sided nasal congestion that no spray can cure.
2. Chronic Rhinosinusitis & Nasal Polyps
The human face contains four pairs of air-filled sinus cavities (maxillary, ethmoid, frontal, and sphenoid). All four sinuses drain into the nasal cavity through narrow microscopic openings called the ostiomeatal complex.
If a deviated septum, mucosal swelling, or fleshy grape-like growths called nasal polyps obstruct these narrow drainage pathways, mucus becomes trapped. Bacteria multiply rapidly in the stagnant fluid, leading to chronic sinus inflammation, facial pain, brain fog, and bad breath.
FESS: Functional Endoscopic Sinus Surgery Explained
Decades ago, sinus surgery required painful external facial cuts, lifting the lip, and extensive facial bruising. Today, Prof. Dr. Maqbool Ahmed Baloch utilizes Functional Endoscopic Sinus Surgery (FESS):
- Zero External Cuts: The entire surgery is conducted entirely through the nostrils using high-definition, micro-optical fiberoptic endoscopes.
- Preserves Natural Mucosa: Rather than stripping healthy sinus lining, FESS gently widens the natural drainage openings (antrostomy) and removes diseased tissue and polyps using precision microdebriders.
- Rapid Drainage Restoration: Trapped mucus and infection drain naturally, restoring normal air circulation and mucus clearance permanently.
Septoplasty: Straightening the Airway from Within
If DNS is the root cause of obstruction, Dr. Maqbool performs a Septoplasty, often in combination with FESS in the same 45-minute session:
- Done completely through an internal incision inside the nostril.
- The deviated cartilage and bone spurs are straightened, reshaped, or repositioned into the midline.
- The inferior turbinates are gently reduced in size (Turbinoplasty) using modern radiofrequency coblation, instantly opening up the nasal airway.
- Zero visible facial swelling, zero black eyes, and zero changes to the external cosmetic appearance of the nose.
Recovery and Life After Surgery
Modern sinus and nasal surgery has advanced dramatically:
- No Painful Traditional Gauze Packing: Instead of painful ribbon gauze, dissolvable hemostatic packs or breathable soft silicone splints are used.
- Day-Care Procedure: Most patients return home the same day or the following morning.
- Breathing Relief Within Days: Once mild post-op crusting is cleared with simple saline rinses, patients experience a dramatic, life-changing improvement in airflow, deep sleep, and elimination of sinus headaches.
