Tympanoplasty vs. Mastoidectomy: What to Do if Your Ear Discharges Continuously for Months

Continuous foul-smelling ear discharge can be a warning sign of cholesteatoma or eardrum perforation. Learn how modern micro-ear surgery permanently stops infection and restores hearing.

Tympanoplasty vs. Mastoidectomy: What to Do if Your Ear Discharges Continuously for Months
Medically Authored & Verified by Prof. Dr. Maqbool Ahmed Baloch · Senior Otolaryngologist & Cochlear Implant Surgeon.

Why Persistent Ear Discharge (Kaan ka Behna) Must Never Be Ignored

In Pakistan, recurrent and persistent ear discharge—commonly referred to in Urdu as "kaan ka behna"—is one of the most frequently neglected medical conditions. Many patients treat running ears as a minor nuisance, treating it periodically with over-the-counter antibiotic ear drops or home remedies for months or even years.

This approach is dangerous. Continuous or recurrent ear discharge is the hallmark symptom of Chronic Suppurative Otitis Media (CSOM). Left untreated, chronic infection does not simply stay inside the middle ear; it can erode the delicate hearing bones (ossicles), destroy the inner ear causing permanent profound deafness, paralyze the facial muscles, or erode through the skull bone into the brain, causing life-threatening meningitis or brain abscess.

In this clinical guide, Prof. Dr. Maqbool Ahmed Baloch, leading Otologist and Micro-Ear Surgeon in Islamabad, explains the difference between the two primary surgical interventions: Tympanoplasty and Mastoidectomy.

Safe vs. Unsafe Ear Discharge: The Critical Distinction

During an ear examination with an operating microscope or high-definition otoscope, an ENT specialist categorizes chronic ear disease into two distinct types:

1. Mucosal / Tubotympanic Disease ("Safe" CSOM)

This is typically characterized by a central hole (perforation) in the eardrum. The discharge is usually mucoid, non-foul smelling, and recurs whenever water enters the ear or during an upper respiratory cold. While termed "safe" because it rarely spreads to the brain, it still causes progressive conductive hearing loss and recurrent infections.

2. Squamosal / Atticoantral Disease ("Unsafe" CSOM / Cholesteatoma)

This is a serious condition characterized by an attic or marginal perforation, often accompanied by an aggressive, destructive skin-lined cyst called a Cholesteatoma. The discharge is typically scanty, foul-smelling, and persistent. Cholesteatoma secretes lytic enzymes that destroy surrounding bone, putting the patient at immediate risk of facial paralysis, labyrinthitis (severe vertigo), and intracranial complications.

Tympanoplasty: Rebuilding the Eardrum and Hearing Chain

When the mastoid bone is not diseased and the primary issue is a perforated eardrum with damaged hearing bones, Tympanoplasty is the definitive surgical solution.

  • How It Works: Dr. Maqbool harvests a small piece of natural connective tissue (temporalis fascia from behind the ear or tragal cartilage) and grafts it under or over the eardrum defect.
  • Ossiculoplasty (Hearing Bone Reconstruction): If the three microscopic hearing bones (malleus, incus, and stapes) have been eroded by chronic infection, titanium or hydroxyapatite micro-prostheses (PORP or TORP) are placed to reconnect the eardrum directly to the inner ear.
  • Success Rate: In experienced hands, Tympanoplasty achieves a graft take rate exceeding 95%, permanently waterproofing the ear and restoring clear natural hearing.

Mastoidectomy: Eradicating Deep Bone Infection & Cholesteatoma

When chronic infection or cholesteatoma has invaded the honeycomb-like mastoid air cells located within the skull bone directly behind the ear, a tympanoplasty alone cannot cure the disease. The infected bone must be surgically cleared via Mastoidectomy.

  • Canal Wall Up (CWU) Mastoidectomy: Dr. Maqbool uses an operating microscope and high-speed surgical drills to open and clear the mastoid air cells while preserving the normal anatomical ear canal wall.
  • Canal Wall Down (CWD) Mastoidectomy: In extensive cholesteatoma cases, the posterior canal wall is lowered to create a clean, accessible cavity that eliminates any possibility of recurrent, hidden disease.
  • Combined Approach: In modern otology, mastoidectomy is combined with tympanoplasty (Tympanomastoidectomy) in a single operative session to eradicate disease and restore hearing simultaneously.

What to Expect During & After Ear Surgery

Under modern microscopic and endoscopic surgical techniques performed by Prof. Dr. Maqbool Ahmed Baloch at PAF Hospital Islamabad:

  1. The surgery is performed under general anesthesia with minimal discomfort.
  2. Patients are typically discharged on the same day or following morning.
  3. A light dressing is worn for a few days, followed by dissolvable ear packing.
  4. Strict water precautions (keeping the ear completely dry with Vaseline-coated cotton during bathing) are observed for 4 to 6 weeks until full healing.
